Read MoreIt’s common for multiple parties to be involved when a patient is going through end of life care. To ensure the patient gets the best care possible, it’s important for all parties to communicate with each other and to have one central party coordinating all these efforts. We are able to use our extensive experience and serve as this coordinating party.
We can help patients and families by working with other care providers to eliminate loose ends and gaps in the patient’s care. To do this, it’s vitally important we effectively and frequently communicate with the family of the patient. This allows us to get feedback on the patient’s changing needs and conditions and keep the family members informed of any changes we plan on making in the care plan.
Family communication also gives us the opportunity to stay in the loop of any other care providers like inpatient facilities, home care agencies, physicians, pharmacists, clergy, and funeral directors.
