Palliative Care

Palliative care is for anyone with a serious illness. You can benefit from it during any stage of illness, and you can receive the care along with curative treatment. It is not dependent on prognosis, and it is not restricted to end- of- life care.

Support includes:

  • Assessment and relief of distressing symptoms
  • Discuss and Manage treatment options with patients and family members
  • Address goals for chronic medical issues
  • Services can be provided at home, in an assisted living, retirement home, skilled nursing center/rehab, Long term care home, residential home or a family residence.
  • Provide guidance with advance directives and/or advance care planning while having goals of care discussion.

Home Health

Services Provided

Home Health Nursing

Our nurses visit patients in their own homes to deliver medical care when a patient is unable to care for themselves. We assess patients' condition during our visits and also check vitals and review medications. Our nurses also may administer medication as prescribed by the patient's physician. If your care requires wound care, we are able to provide that as well, including dressing, redressing, and assessing the progress of healing.

Home Health Aides

Our home health aides are trained to assist patients with activities of daily living such as eating, bathing, grooming, and taking vital signs. Under the supervision of our nurses and therapists, home health aides also support certain medical care activities like taking a patient’s pulse or blood pressure

Physical Therapy

The goal of our entire team is to provide the best and most supportive services while helping clients to regain control of their lives. Often time when tragedy strikes, overall mobility can suffer. The physical therapists on our team are prepared to assist you or your loved one in overcoming these obstacles and regaining their control and confidence. 

Occupational Therapy

Occupational Therapy can be viewed as one of the pillars of in-home care overall. Our occupational therapists can be pivotal in a patient's overall success in recovery and regaining independence. Some areas that we evaluate and treat are neurological, musculoskeletal, orthopedic, fine motor skills, and more. 

Speech Therapy

Speech therapy focuses on helping patients maximize their skills and abilities to live independently. Our speech-language pathologists are experts in oral and motor cognition and help patients improve their swallowing, mental processing, and language skills. 

Medical Social Work

Our social workers assess the social and emotional factors affecting each patient and their loved ones and then assist in coordinating available community resources and short-term counseling.
